Car dies in warm weather (97 A4 2.8 V6-12v)

Car dies in warm weather (97 A4 2.8 V6-12v)
Hi everyone,
As the title states, I have a 97 Audi A4 2.8 V6 12v that dies every once in a while. I first noticed this problem last summer, but I ended up not looking into it much because when winter came around the problem went away completely. Basically, what happens is when it gets above around 60 degrees outside the idle will drop really low then immediately shut off. If I feather the gas pedal when stopped this will often prevent this from happening. The only way to restart the car is to either spray a little starting fluid into the throttle body (I know this is not good for it) or wait about 20 minutes. Before the car begins to start acting up it usually jolts really hard (feels like engine turns off then suddenly starts up again) and throws a CEL.
I currently have the following check engine light codes:
P0103-Mass Air Flow Circuit 1 High Input
P1504-Intake air system leak detected
P0121-Throttle Pedal Position Sensor/Switch A Circuit Range/Performance
P01250-Fuel Level Too Low
What is odd to me is that the car works great in cooler weather even to the point where it does not seem to have any problems and that it never has a problem starting when you first start it up no matter what the temperature is outside.
Can someone help point me in the right direction?
