Applying the brakes while starting. I am in the habit of putting my foot on the brake before turning the ignition on. Part of the process I worked through was that the problem mainly happened was when I had my foot on the brake pedal when turning the ignition on. When I didn't touch the brake pedal, the car would start then drive OK.
