Fan Bearing?

Fan Bearing?
I'm not 100% exactly sure if this is the problem. But when I first start the car and the fan comes on it makes a loud sound like a bearing is worn out. After about 2 to 3 minutes after the car warms up the sound is gone. If anyone could help it would be greatly appreciated. Thank you very much.
