shifter rod coming loose

Default shifter rod coming loose

Okay so I have an 01.5 s4. put in a new rs4 clutch and alu. flywheel. put everything back together no problems and no random parts left over. get it 2 miles down the road and have no gears. had to tow.. get it back to the shop and shifter rod nut coming out of the trans is loose. cranked it down. took it about 100 miles same thing happens. Am I missing a bushing? or overlooking something simple? any advise helps.

actually just found the missing piece. It's a little wedge shaped piece of metal a little bigger then my thumbnail. It's not supposed to come off but if you can find it, it can be welded back on.
