Summary: I ordered some blizzaks for GF's honda, forgot to print out American Express rebate form, now I cannot find it on your site. Can you email it to me?

The rebate form and invoice are mailed automatically to your billing address, typically within two weeks of the tires arrival. If this hasn't arrived, my customer service dept can assist in either getting another form mailed out or sending you one electronically. They can be reached at 1-800-428-8355, ext 360.
