Probably changes to valve timing, head shape and manifold porting plus significant changes in the software mapping of engine parameters. All the 3rd party tuners show the tuning potential of their remapping software. The original car manufacturers have to be somewhat conservative with their mappings since they all have an explicit factory warranty they have to provide. If a stock engine fails inside the factory warranty time, it's clear who is responsible. If a failure occurs Y months after a 3rd party tune is installed, who's accountable - that's the very grey area and why Audi implemented their TD1 process.
The 3rd party tuners do a fantastic job (e.g. ECS from previous post) of showing what smart remapping can achieve without placing excessive additional stress on the stock engine. As long as you follow their guidelines for the various stages of upgrade, we can all get to enjoy additional performance at reasonable cost.
