Default 3.2L Crank Pulley failure

I was just getting near home when I heard a noise up front, lost my power steering and had the battery light come up. I figured the car tossed the belt for some reason. Upon looking closer, my crank pulley has torn off. There are only 3 mounting bolts left on the crank (all right next to each other), and looking at the pulley, there is one hole that didn't rip off. My guess is most of the bolts backed out over time for some reason, and the pulley fatigued with just 3 maybe 4 holding it in place. I haven't looked closer at the crank yet to see if any bolt heads sheared off and are still in the crank. I'm letting the car cool down some more before investigating more. I did notice some minor vibrations at idle when coming home from work yesterday. I originally thought it had something to do with the ignition, now I figure it was this. The car is an 05 with just under 78k on it. I've had it since 2012 and 29k. I've had all regular service done by the dealer except for me doing other things such as doing the brakes and swapping tires. As far as I know, the pulley area has never been worked on.
