Obviously the 19" wheels looks much better and fill out the wheel well better, but I don't like the extra money they are going to cost (repair, tread life) and the ride quality vs. the 18". Are they absolutely necessary for a new S4?

No, other than the looks, the 18's are probably a better choice for many of the reasons you just listed.
