vertical mount 150mA, but I could be wrong....plus, measure the pin spacing and dia. first,...

I might be able to find one lying around work and mail it to ya.

Just opend mine. It's a TR5 package Quick Acting 125ma 250V fuse.

Here are the markings on the package.
