Had some strange DTCs today... Anyone shed some light on them?

Had some strange DTCs today... Anyone shed some light on them?
16891 P0507 Idle Control Module - RPM Higher Than Expected A
16786 P0402 Exhaust Gas Recirculation (EGR) - Flow Excessive B
16542 P0158 O2 Sensor Circuit - High Voltage (Bank 2 Sensor 2) A
I just had this 02 sensor replaced, fortunately they only showed up as a sporadic DTC with the VAG1552... I cleared them, but anyone know what would cause them? I've only recently been having these O2 sensor errors(within the past 3 weeks) - I'm pretty sure the High voltage error is due to my car running rich (which it is) - could this be a sign of possibly a leaking BPV again? Or elsewhere in the plumbing?
