Very difficult to get into gears after the car warms up... When I floor the car in higher gears...
the rpm would jump really high then drop back down. And the clutch feels like its not grabbing as well as it used it. Is the clutch going out? Is it the synchro? or could it be both?

This could be your diaphragm spring in the clutch going out. It happened to me. I didn't get
slippage, but once the car was warmed up I couldn't disengage the clutch at all. Your's could be a variation of this situation. Before it went out totally, it was almost impossible for me to shift smoothly.
