Default Problems with high revs

My 2.5 TDI has an annoying problem.
Some times it feels like the car is pushing the throttle while I'm not.
This results in high revs, typically 1200 or 1400.

The issue here is that it is hesitant to change gear and rather go 3000 rpm than change up, and also when I'm accelerating it changes gear but doesn't match the revs, so it rather goes to the limiter and then releases the clutch, which I cannot imagine is good for the car..

I have no idea what this may be, and it seems to be worse when its moist outside, though this may just be my imagination.

It feels like the car is pushing the throttle for me, and this adds to what I do. What can this be?
I did check vagcom earlier and don't think there was any errors, but I may check again.

This is a 1998 A8 2,5 TDI Quattro with tiptronic gearbox.

have you been going through oil? Does it smoke when this happens? This sounds like "diesel runnaway" Its from the engine somehow sucking oil somewhere and using the oil as fuel, the engine speed is detirmined by fuel, more fuel, more throttle. So since diesel is pretty much oil, the engine oil burns. List includes Bad turbo, bad valve seals, bad rings, Somthing wrong with the crankcase breather system, intercooler full of oil from turbo, or even way over filling the oil.
