fuel top-off & rough idle?

fuel top-off & rough idle?
If I top-off I get some hesitation at idle and some mildly irratic revving. Am I blocking a vent/vacuum port by nursing those last drops into the tank? Or is this:
a) pre-failure warning sign,
b) indicative of something else?
