Yes, your dealer can order one configured the way you did it on the Audi USA website.
It's a slow process.
Since you are on the east coast that helps, but you are still looking at about four months from order to delivery.
As I have advised before, if ordering an Audi ask your salesperson for a "Vehicle Details" printout.
This shows all the relevant information about your car and it's projected dates, this is very important.

1. When does one pay for an order? On pickup?
2. Do you put down a deposit? Non refundable?
3. ...Or have I heard that you could back out? (Not that I would plan to.)
4. Presumably you need to negotiate the price of the car (and any discounts) PRIOR to ordering?
5. And, if any sales incentives are available at the actual point when your car arrives, you can take advantage of them?
6. but, what if the car arrives just after a sales campaign ends? SOL?
7. And any trade valuation would be at time of pickup... So you are rolling the dice on what they might give you for your trade?

When my sister ordered her previous TTS, she placed the order in early March and got the car in late August. It took about 3 months for the order to be accepted by the factory an then another 3 months for the car to be built and shipped over.

Yes, balance is paid upon delivery.

Yes you can back out. Note: this is why some dealers will balk on very unusual option orders as they are concerned with getting stuck with same.

I "pre-ordered" my TTS in June 2016 and took delivery in late November. Although I didn't put huge miles on my trade-in over the span the trade value at ordering was still honored.

Seems supplier and ACNA discounts are not available on RS models. My dealer said he would do 2% on a TTRS. Which leaves the trade in as the biggest negotiable (if I go the dealer trade in route). Seems, for RS... If you want the car, you gotta pay. :-)
