set the car down off the jack and let the weight of the car break the wheel loose.

They can corrode together over time.

yes, coat hub with some anti-sieze before installing wheels

grab tire with both hands 180 degree apart, rock, rotate 90 degrees>

repeating .... usually i can dislodge the wheel by the time I make one revolution of the tire .... you really gotta put some muscle into it .... so make sure you have it on jack stands.
