4.0TDi, Hole in head between exhaust port 2 and 3.

4.0TDi, Hole in head between exhaust port 2 and 3.
Hey,
Bought a 60k mile dark purple preface, drove it 100km and started hearing a pretty hefty exhaust gasket leak. Thought to myself, its a shame to pull the engine for this but could be worse.
Borrowed a borescope to get a better look, saw a pretty sizeable hole in the head between the exhaust ports. Got pictures if anyone is interested.
Does anyone know what caused this, cant seem to find any info on this odd issue.
Ive heard its caused a some type of gasket related to the injectors and a canal wich goes out by the exit ports.
If anyone got some info on this that'd be highly appreciated.
