installing an arm rest, looking for FAQ

How is your arm positioned during shifting?
A) Just your fingers on the gear shift, fingers facing down;
B) Palm of hand on top of gear shift, like you're rowing a boat;
C) Palm of hand on side of gear shift, like your're holding a baseball bat.
D) Other
I can't comfortably shift into or from 2nd or 4th with the armrest in the full down position. I'll have to take a few pictures to show what I mean.
