What's the difference between the KO4 and the T28

different turbos, different manufacturer.
KO4 is small, T28, especially the trim of the PES T28, is a pretty large turbo. KO4 good for about 20psi, the T28 is good for 26+psi (of course dependent upon a variety of factors)
Attached is an old sig of mine which has a T28 and a KO3 (which is same housing as a KO4) sitting side by side:

Thats like saying whats the difference between an microlite and a 747 !
Its bigger!
Things to consider are, bigger = generally more lag, generally higher RPM req'd for max power.
This means > faster, but in general less comfortable for day to day driving.
The alternative to the T28 (PES) is the Garrett GT25 (APR), which is generally used in ball bearing mode, which allows it to spool faster and sooner than traditional mid size turbo's.
(PS, I've just consumed a bottle of wine, so I hope I'm correct!)
