When tie-rods go bad... do we replace the tie rod ends or the tie rod assembly?

When tie-rods go bad... do we replace the tie rod ends or the tie rod assembly?
ECS has
"OE Tie Rod Assembly (inner & outer ends)-Priced Each, Left or Right" for about 100 ea.
and
"OE Heavy Duty Tie Rod End Set, Left & Right Ends (Pair)" for about a 100 per set.
So which one is it that I should get? Or does that depend on whether it is the tie-rod ends that break or the entire assembly?
Also, would the labor be much higher if I am just changing the "tie-rod ends"? Would it be difficult to attach the tie-rod ends to the tie-rod assembly?
Sorry, I am super-newbie at this, and read a post or two up on why I think my tie-rods are dead.

get the heavy duty tie rod end set
and to know they're dead, it'll squeak at low speeds when turning. like when you park. i did mine last week..world of difference. steering is much tighter and the sounds are gone

So getting the entire assembly is probably not required, right? Thanks for telling me about your experience...
