It looks like one could just add the paddles to the 4-spoke, but it would require changing out the harness.
ETKA implies the 4-spoke paddles are 4E0 951 527 H and J, vs. 4E0 951 527 K and L for the 3-spoke sport wheel. I'm not sure what exactly would be different.
Switching from the 4-spoke (PR 2ZQ) to the 3-spoke (PR 1XW if no paddles, PR 1XX if paddles) seems there was also a change of airbag module. I'm not sure what exactly is different; the round issue doesn't come into play till B8.5.
The no paddle harness is 4F0 971 589C, vs the paddle harness of 4F0 971 589B.
There is a 4-spoke paddle wheel on ETKA, but not under US spec. It's PR 2ZS, seems to only have come on lane keeping system equipped cars. It uses a different harness of 4L0 971 589. So I'm thinking that's about the lane keeping system (rocks the wheel), and just adding paddles to a US 4-spoke would just need the B rev harness.
The 4-spoke paddle part numbers are associated with B7 years A4, and A8. They used 4F0 971 589A, replaced by 4F0 971 589K. I'm not sure what's actually different about K vs B. A (thus K), appears to have the two plugs for the paddles.
If it was as simple as just getting the two paddles, the harness, and the two screws for the paddles, I'd add that to my 09 without a second thought.
Getting a sport wheel with paddles (8T0419091B) is not cheap, and the used stuff seems to often be disassembled (missing paddles, harness, etc).

Ah, nevermind. I finally got that video playing and I see the screw I was looking at is not the screw that holds the paddle in place. So yeah, you'll need a different wheel, one with the openings for the paddle. As noted here https://www.wolfautoparts.com/media/...8226.JPG_1.jpg
That sucks. For the US, that means having to go three-spoke, which means needing a different airbag unit apparently, and is likely near impossible to find used in beige. For that kind of money, I'll stick to the gear shifter.
