My extended warranty just covered $800 of front-end work that my '01 ar needed (rods, tie rod ends, etc. and labor). I spent just under $3,000 for 150,000-mile warranty and it's already covered nearly $7,000 in repairs. I've got a third engine leak (very minor) and I'm told that it'll be covered as well.

My second best investment after the car was the policy. However, I'm not sure if 150,000-mile warranties are easy to find these days and many of the problems popped up after 105,000. Meantine, the car's still running beautifully.

Check the archives. I've posted their website and other info over the past few years. They're far from great (they can bust ***** in terms of "usual and customary" costs for repairs but, overall, they're pretty fair.
