Default Clunking from left rear shock

About four months ago I started hearing a clunking noise from the left rear of the car. It turned out to be a bad Bilstein shock. Luckily it was replaced under warranty. It's doing it again, so I thought maybe a bad shock? I replaced it but it didn't help. It appears that the top shock shaft that the nut attaches to is coming up and hitting the area directly above it. I looked and there is a rough looking hole there. Has the shaft made that hole? Or was there a hole there and the shaft is making it appear rough? What would make the shaft hit that area?

Check if the top nut that's securing the spring is tight.

Tightened down and bottomed out, checked twice.
