new '04, a6 avant; non-bose:

1) want to replace front (horrible) speakers in lower door panel. What is correct size and does anyone have suggestions for these? (I would much rather NOT change out the head unit.)

2) car has 'sat' button; but i dont have sat. radio. I know 'Infotainment Package' comes with everything standard (which i dont have) but can XM or Sirius also be added as a dealer installed option?

You can most likely add either XM or Sirius. I'm not sure at this time which vendor Audi supports, but both are pretty good stuff.

If you want to have a clean, factory look, the "factory installed sat radio option" can be added after delivery. It is a very simple install which (in the A4) involves mouting the receiver box in the trunk and swapping a connector up front somewhere. A couple of people on the B6 A4 forum have done it. I'm sure it's not too different for the A6. You might want to do a search for instructions because somebody posted a step-by-step guide w/pictures.
