You don't have to do it now....
You can add it at any point before the factory one ends. If you are 100% sure your going to keep the car past the 4yr/50k then go for it now, if you might sell I wouldn't bother now.

ask one of the service writers at your dealership....
...they're usually the ones who have to call to get work pre-authorized under any extended warranty, so they'll know which ones are good. When I had my A6, they recommended one called Mastertech or something like that....in 2 years, they covered enough repairs to pay for the extended warranty twice (and none of the repairs were 'major'...just several little things).
