I think that is the right link, but...
It looks likes it may be by invitation only. That's the second link posted, but with a different emid. Just for fun, I changed it to 9999 and it went to a screen at the beginning of the query.

OnOlRx7 is right, here's how to take the survey.
Use the link that's already been supplied, but change the last 4 numbers to any combination of 4 numbers you want. Then hit go. As long as they're numbers no one else has used yet, you'll be taken to the beginning of the survey. It seems that once the survey has been taken by a specific "emid" number, using the link takes you to the END of the survey. Use new numbers, and you'll start at the beginning.
Personally I thought it strange that when asked about how much you've spent on accessories, there's no "$0 - $500" category to choose. It goes from "N/A" to "$500 - $1000". Very sloppy.<p>Rick
