What is the highest horspower turbo kit for the a4? sIII+? or can i go custom w/ a huge turbo?

There is no stage 3+ at this time
You can go with a PES T28, Apr Stage3. ATP is still working on their kit that uses a T3/T4 and Werks of Art is also working in 2 new kits, one use a very large K04 that bolts right to the stock manifold which should put out 260 hp and a full kit that is much larger that should make 350+ hp. If you plan on going custom be ready for long days and nights of tuning, the turbo and parts is the easy side of it.

They dont make a fmic so how could they sell a stage 3+.
That is one of the reasons they stopped selling it the other reason is that they claim 310 hp with a race chip on the stage 3 which would make selling a stage 3+ at 310 hp for another $1K a little hard.

MTM uses very large 3k turbos
Not sure how they plan to make good power in the lower rpms and still make 330 hp and 420 hp. Here is their prices which I am pretty sure are 3-4 times as much as a PES or APR kit.

My guess is that they would offer a 100 and 104 program for the 3+ as well so base HP isn't an issue
280-290 base and 310 on the 100 program vs 310 base and 340-350 on a 100 program for the 3+. IIRC it is the FMIC and the fact that the Garrett turbo itself became hard to get. Of course I don't speak for APR but my understanding is that now that they are a Garrett 'partner' they can get spec'd turbos again, do the in house set up etc. So, I think it comes down to the FMIC and software updates.

No one really talks about mtm around here. Maybe it's just too expensive like you pointed out.
They should run quite nicely on low rpms, atleast according to graphics you see on their website. Well I really don't care too much cause I ain't going to uppgrade my audi to 420 hp...
