This is my gear changer which looks a mess, are they easy to change? Just pull up?

Nope, it is that easy. The leather part is snapped in place, with a star bit screw holding in the actual lever, IIRC. It is very easy to remove.

The only thing now is finding a decent one on fleabay that isn't left hand drive

I've yanked my gearshift leather top off a few times, just to see, very easy. My leather is peeling off from my watch scraping it - I wear mine on the right hand, which means I'm nuts obviously

Can you buy the top part only from the dealers or is it the whole thing? How much?

You can buy just the top leather bit. I found it brand new for American spec cars for $131. I'm not sure if it'll work for you or not. But here it is with the part number.
