You need to fill out another form...
It's just like a declaration document, that specifies the contents and their value. USPS has the same deal. I actually found that USPS shipped faster sometimes, but not sure if that was a total fluke.

You need the following ; please use USPS as UPS will overcharge brokerage fees BIG TIME.
You need a "commercial invoice" (even if it's non-commercial). You can take the form from any transporter, it doesn't matter...
Here is a link to one commercial invoice (.PDF) : http://www.aircanada.com/en/fedex/CI_Form-Schedule_D.pdf
If you're selling used stuff to somebody, I suggest that you put the declared value as low as possible, for the buyer. $20CAD or lower, you don't have any brokerage fees... Above that, the buyer will pay 15% taxes approximately + a brokerage fee (10$+). UPS is the worst, I mean it. Don't use them to ship to Canada. Use USPS, it will cost 4 times less in term of brokerage fees...
Write the following:
Invoice Number : Non-Commercial
Terms of Sale : Non-Commercial
Related Parties : No
... and don't bother for the "international way bill number" if you don't ship with FedEx...

It's not a big deal, not worth going by usps at all. actually, no difference when you go to a ups
store. we just ask you whats inside and you need a value and a phone number for the recipient. we fill out the other form and send it with the required 4 copies. to cut down on duties, we usually say it's a gift. even if there are duties, the recipient pays them, after all he is buying something from you, just like i'm sure he pays shipping.

Big Big 2nd... and 3rd! and 4th.... UPS are bunch of crooks for brokerage fees a few weeks later...
I once received, 4-5 weeks later the reception of a package from USA, a $71.55 (to be exact) bill for merchandises worth $220... Bunch of bastar%s...
