If you replaced all the chains and tensioners, then your camshafts might be slightly out of time or something worked loose during the re-assembly. But there is also the possibility that the upper chain tensioner(s) may have failed.

In order to fix the issue, I need to replace the upper chain tensioner on bank 2?
