Yes, they are attached to the steering wheel. I feel it hard to reach to the paddles sometimes so the extender makes it easier to reach.

The paddles are attached to the steering wheel. They move with the wheel.

People install paddle extensions because they want to be able to reach the paddles from different positions on the wheel.

It's not a "need" but a "want" and it's really very simple. If your hands are at 10 and 2 on the steering wheel, unless you have really long fingers, you must move your hands lower on the wheel to reach the smallish paddles. The extensions, which are fastened to the paddles, provide extra length to activate them from higher (or I guess lower) on the wheel. If you always keep your hands at 9 and 3 on the wheel, no extensions are necessary since the paddles are right there. I'm a mostly a hands-on-the-wheel-at-10-and-2 kinda guy and as I don't have 5"-6" fingers, I find that the extensions are very useful. Nothing more complicated than that.

Ahhhhh makes sense.

It would definitely be tough to reach the paddles from other positions on the steering wheel. Those extensions do look nice

Although some would maintain that "spirited driving" should always be 9 and 3.

True, but I use the paddles frequently to downshift even with relaxed, casual un-spirited driving as well. One hand at 10 and one hand fiddling with the MMI.
