B5 S4 Avus is ET45 only for real versions. It works fine - I'm using them now with a 10mm spacer.
I put it on mainly cause I wanted it to sit correctly in the fender. Without the spacer it still fits, and even clears the Big Reds... but it sits a little bit too far in for my taste.

Correct - you should get wheel bolts to match the spacers. Most spacers can come with them.
Contact "Brian" at www.northeuropeanautomotive.com if you need suggestions. I got mine from them and the total came to right around $200 including wheel bolts.
