Leak anywhere between the maf and throttle body.

A maf going bad will also cause a lean issue.

look at your fuel trims

then you can at least tell under what condition the lean condition is happening. Might help pinpoint the problem.

Make sure the codes haven't been cleared for a while before you look at the fuel trims. It takes them a day or two to get dialed in sometimes.
