I want a Q5 with the 2.0T and a six speed manual

And even BMW's "enthusiast" version could stand to lose 200 pounds.
The X3 is not exactly light. Coming from a 3-series coupe (and an A4 before that), I can definitely feel the extra heft. It's still much more fun to drive than my parents' Allroad 4.2Q though.

nobody, because there is no market for it.
i'm surprised BMW sells it- maybe just for bragging rights. Even in Europe, manual transmissions are becoming more rare on larger luxury cars. Of course, they get the Q5 with DSG there.

Unlikely
Even here in the UK - where over 90% of cars are manual - the 2.0T is S-Tronic for the foreseeable future. And the engine is expected to be so marginal in sales that no dealer currently has a demo available AFAIK.
Manual of other engines are more likely.

The elephant in the room on MPG is IMHO
You have to drive a 2.0T like your driving your grandmother everywhere to achieve those figures. Break 4000 rpm and it's like someone put a screwdriver through the bottom of your fuel tank!
I average 26.6 mpg (imp) in the A3 2.0T and my wife gets ~28 in her GTI. Now admittedly we are talking the old belt driven engine and efficiency is better on the new engine, but I bet you don't see much real world difference.
